DBS (Disclosure and Barring Services) Application forms
All drivers must be enhanced DBS checked every year with effect from 1 January 2016.
Alternatively, a driver may apply annually for a DBS check by completing and submitting the DBS form and supporting evidence with the fee of £44.00 to the Licensing Office.
DBS forms are available from the Licensing Office.
Please note that the DBS application form, once completed, must be submitted in person to the Licensing Office together with the supporting methods of identification.
Please note it is the driver's/applicant's responsibility to ensure that an annual dbs check is recorded with the licensing office as failure to do so will result in suspension of licence until such time as a dbs check is carried out.
